Patient Referral Process

Visit with your child’s physician about your concerns and request a referral for Occupational Therapy, Physical Therapy and/or Speech Therapy.  Your physician can fax the referral for the Sheridan, Cheyenne, Gillette, or Casper office to: 307-683-4005

Call us to schedule a free phone consultation or an appointment with a therapist.

Once referred, please download, print, and fill out the patient forms below and bring them to your appointment.

We can bill private insurance as well as Medicaid.  Discounted rates for prompt pay at time of service. We also accept credit cards.
